The ending for the 2005 Royal Rumble, where both Batista and John Cena went over the top rope simultaneously only to have the match restarted and eventually won by Batista, was not planned. Batista tripped and fell along with Cena, but was scheduled to win cleanly.

Benoit Vs. Triple H Not On For SummerSlam?
Despite Triple H's obsession as of late with regaining "his" World Heavyweight Championship from Chris Benoit, it appears that it will not happen. Instead, it seems as if WWE are keen on taking completely different routes for both wrestlers.

It is rumored that Triple H might go on to wrestle Eugene at the August 15th event, while it also seems highly likely that Benoit might be defending his World title against the former Intercontinental Champion of Randy Orton. Rumor also has it that WWE are quite keen on putting the World Heavyweight title on Orton in the near future, but that could also change.

Still, nothing has been officially confirmed at this point in time.
